This service is available to anyone under the age of 18 with a referral from a physician.
- Please contact your Primary Care Provider to request a referral if required.
- Please bring your health card to every appointment.
- Please arrive on time.
- Check-in with the admin clerk upon arrival and wait in the waiting room until the nurse is ready for you.
