Primary Care Advanced Support Program

The Primary Care Advanced Support Program is a reimagined and enhanced approach to delivering comprehensive, patient-centred care for individuals with complex health needs and chronic conditions. This program is not new, but rather a thoughtful rebranding of the Congestive Heart Failure/Family Health Worker RN service, with a renewed focus on supporting a wider range of patients and conditions.

In this program, our dedicated team of nurses works closely with your family physician or Nurse Practitioner to provide personalized care and support in the comfort of your own home. Our goal is to empower patients and their families to maintain their independence and stay in their homes for as long as possible. By collaborating with other healthcare professionals, we ensure that all aspects of chronic illness management are addressed to prevent hospital admissions and promote overall well-being.

This support team is comprised of Registered Nurses (RN), who provide comprehensive, patient-focused care for individuals with complex health needs and chronic conditions. The Primary Care RNs are a dedicated team committed to patient-centred care to support and guide patients and their families in their health journey.

This service is available to Group Health Centre rostered patients and Acess Care Clinic registered patients. To access the Primary Care Advanced Support Program, patients must be referred to the program by:

  • Primary Care Proviider
  • Self-Referral

Once the referral has been received, our team will contact you to book your initial appointment.

Visits usually last at least one hour. Your Primary Care RN will review your medications and check your vital signs at each visit.

November 29, 2024 – Due to severe weather conditions, GHC will be cancelling all in-person appointments today. We will be contacting all patients with appointments scheduled for today to reschedule or transition them to virtual care where possible.
