Algoma Diabetes Education & Care

GHC’s interdisciplinary team of Certified Diabetes Educators works with your primary care provider to support both adults and children with; Type 1 or Type 2 diabetes, gestational diabetes, and prediabetes.  

The team includes registered dietitians, registered nurses, a social worker, and a foot care nurse.

ADEC promotes patient self-management and provides ongoing support to individuals, their families, community agencies, and healthcare providers in preventing and managing their diabetes and those who are pre-diabetic.

ADEC offers the following services for adults and children across the Algoma District:

  • Insulin/oral medication adjustment (with a signed medical directive)
  • Insulin starts; basal/bolus insulin therapy
  • Insulin pump program
  • Foot care and foot screening
  • Assistance with continuous Glucose monitoring
  • Classes on a variety of topics, including stress/relaxation, cholesterol, and label reading
  • Community health promotion activities and diabetes awareness to service groups, schools and other healthcare agencies
  • Individual or group appointments for clients who are newly diagnosed with diabetes or prediabetes

Frequently Asked Questions

  • ADEC is open to any member of the Algoma District diagnosed with diabetes or prediabetes.
  • While a referral from a Primary Care Provider/Health Care Provider is preferred, self-referrals are also accepted.

Note: Clients without a Primary Care Provider may be required to visit a walk-in clinic if follow-up medication or further blood work is required.

  • Once a referral is received, a member of the ADEC team will contact you to book a one-hour appointment.
  • On their initial appointment, clients are required to bring a three-day food log.
  • On any follow-up appointment, clients are only required to bring their logbook and meter.
